CKD Lab Tracker

Track your labs over time โ€” trends matter more than any single value. Bring this to every appointment and ask your doctor for a printed copy of your results!!

๐Ÿ’š Remember: Your labs are not a report card โ€” they are a roadmap. One result does not define your journey. Look for the direction over time. eGFR trending down over 6โ€“12 months tells us more than one reading. Potassium trending up over 3 appointments tells us something different than a single elevated result. You cannot eat for your labs if you do not know what your labs are.

Lab ranges can vary by lab and by individual โ€” your doctor's targets for YOU may differ from the general reference ranges shown above. Do not make significant dietary changes without guidance from your healthcare team.
